Intake Form

Long time users of eCandidus™ know that we do not have an intake form.  At least not until now.

Following the tradition of giving you the tools to do what you want, you can now create your own intake form:

  1. Scan your current intake form.  Tthe best format is TIFF but PDF can also be used
  2. Create a new form by selecting Forms and then Add in the main menu
  3. Bring in the scanned image by clicking on the Refresh button (the green circular arrow) at the top of the form
  4. Add all of the fields by right mouse-click on the form at the point where you want the field aand selecting Add Field
  5. Move and resize the field by using the Ctrl or Alt keys along with the arrow keys.  Ctrl resizes, Alt moves
  6. Add the merge codes for each field.  Simply drag and drop from the case screen info buttons to the form fields
  7. Make the form an input form by clicking on the Tools button (the cyan ruler) and selecting Set as input form and then Interactive
  8. Save the form, giving it a title and setting the merge if field found under the Info tab

Now you can use the form as you would any merge form, except that when you fill any blank field and the Save the form, the input is placed back in the case.

And there you have an input form. 

By the way, there are no limits to the number of intake forms nor to which fields that you need to fill in, so you can create a special intake form for each injury or vehicle details!  And now with the addition of Dropbox support, you can send the input form to your iPad and work on it completely offile, synchronizing back into the eCandidus™ database when you are back in the office.
